  • Details on medications used in treatment and their possible side effects
  • Management of non-motor symptoms, such as: Constipation, Swallowing difficulties, Neuropsychiatric symptoms, hypotension (a drop in blood pressure when changing position), Sleep disturbances

This diary serves as a daily symptom tracker, allowing patients to record: Daily symptoms and Medication schedules.
